General Information:
Please arrive 15 minutes early for all appointments. To make good use of your time, you can update all pertinent information online in your patient portal.
Please remember that appointments can be made and cancelled online. There is a 24 hour cancellation policy. If you cancel your appointment after 24 hours or no-show (you simply do not show for your appointment), you will be charged $50 to book a future appointment with us. This is not a charge to your insurance but to you individually. Arriving late for your appointment runs the risk that your appointment will have to be cancelled and rescheduled.
It is important that you do not run out of your scheduled prescriptions so please plan accordingly to avoid this. Please attempt to get your refills done during your scheduled appointment. If you need a refill prior to your next appointment, it is best to submit the request for refill through your pharmacy. Please allow 48 hours for staff to respond. Refills will NOT be placed after hours.

Insured Patients
Please ensure that your insurance information is up-to-date with our clinic at each appointment and notify us immediately if there is ever a change.
Please bring valid identification and copay to every appointment.

Uninsured Patients:
We also see uninsured patients. You are expected to pay in full the cost of the visit.
Your right to a “Good Faith Estimate”: Since January 1, 2022, if you’re uninsured or don’t plan to submit your claim to your health plan, you have a right to receive Good Faith Estimate for the total expected cost of any non-emergency items or services. The estimate isn’t required to include items and services provided to you by another provider or facility, but you can ask these providers or facilities for a separate estimate.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
